Wraparound Service Coordination (Referral Support)
D&G RES CARE provides housing only and offers wraparound service coordination to support resident independence. We assist with coordinating referrals to licensed third-party providers and community-based services.
This may include referrals for benefits support, healthcare services, behavioral health resources, transportation providers, and other community supports that help residents remain stable and independent.
HOUSING FEATURES & LIVING ENVIRONMENT
D&G RES CARE provides independent and semi-independent shared housing in a safe, clean, and structured home setting designed to support long-term stability and community living
Homes include:
- Fully furnished shared living spaces
- Private and shared bedroom options (based on availability)
- Shared kitchens and dining areas
- On-site laundry facilities
- Utilities, Wi-Fi, and household amenities
- Clean, well-maintained common areas
- Structured house guidelines and community expectations
- Quiet, respectful residential environment
- Secure access and safety-focused home layouts
Our homes are designed to feel like real homes, not facilities, supporting independence, routine, dignity, and community connection.
ELIGIBILITY
Eligibility Guidelines – I do like the current version and wording. we can keep that but add more info on the verifiable monthly income

- Independent or semi-independent living capacity
- Ability to live safely in a shared household environment
- Community living readiness and stability
Not a fit if:
An individual requires on-site caregiving, hands-on assistance, or continuous supervision from the housing provider.

Monthly housing fees cover lodging and shared household amenities only.
Services such as healthcare, personal support, and transportation are arranged separately through third-party providers.
